PFY’s Privacy Policy

 

This privacy policy discloses the privacy practices for PFY’s agency website, liccpfy.org. This privacy notice applies solely to information collected on this website via the forms. 

 

A. INFORMATION COLLECTION AND USE 

PFY is the sole recipient of all information collected on forms within the agency website. PFY only has access to and collects information that clients voluntarily provide within the contact forms. PFY will not sell or rent this information to any other party or organization. PFY will use all information collected to respond to clients regarding the reason for initial contact. 

PFY maintains HIPAA compliance throughout all interactions on PFY’s sites both online and in-person.

B. SECURITY 

PFY takes precautions to protect all clients Protected Health Information (PHI). When a client submits PHI information via the agency website, that information is protected both online and offline. 

When PHI is collected, it is only distributed to the designated staff members who facilitate the programs and services clients are inquiring about.
